Where do I upload the generated robots.txt file?

Place it at the root of your domain so it is reachable at yoursite.com/robots.txt — subdirectories are not read by crawlers.

Can robots.txt keep pages out of Google entirely?

Not reliably. Disallow only blocks crawling, not indexing of URLs discovered elsewhere. Use a noindex meta tag for pages you need fully removed from search results.
